Manpower is the main asset for the company in driving the company's operations to achieve high efficiency and productivity. As technology improves and industrialization in companies is often accompanied by increased risks and hazards in the workplace that can cause accidents for the workforce. One of the efforts to prevent work accidents that has been implemented by the company based on Government Regulation No.50 of 2012 is a hazard reporting procedure called JSO (Job Safety Observation) and ADA! Card. This study aims to determine the implementation of unsafe action reporting and determine the correlation between the findings of unsafe action reporting and the root causes of accidents at the Spinning department in 2023. The data in the study is secondary data obtained from internal company data, namely the JSO daily report and RCA Accident record in 2023. Based on the analysis conducted, it was found that the implementation of unsafe action reporting in the Spinning department had met the monthly target of >50%. The highest type of unsafe action reporting findings were obtained by PPE violations by 87% while the highest cause of accidents was caused by complacent by 67%, this shows no correlation between reporting and the cause of accidents.
