Gambaran Surat Keterangan Kematian DI RSUD Tengku Rafi’an Siak Pasca Diberlakukannya Pedoman Pengisian Surat Keterangan Kematian Tahun 2016
The life of an individual begins with birth and ends with death. Death is the permanent cessation of life signs, which is followed by a series of administrative procedures and handling of the deceased, one of which is the issuance of a death certificate. The death certificate (SKK) has been regulated in the 2016 Death Certificate Filling Guidelines issued by the Ministry of Health. The purpose of this study is to assess the conformity of the SKK format used by RSUD Tengku Rafi'an Siak with the Death Certificate Filling Guidelines by the Ministry of Health and to evaluate the informants' knowledge regarding the guidelines and the process of issuing the SKK. This study employs a sequential explanatory method, which begins with quantitative research and is followed by qualitative research. The research duration was 12 weeks. The sample size consisted of 143 SKKs from 2021-2023, followed by in-depth interviews with individuals involved in the creation and issuance of SKKs at RSUD Tengku Rafi'an Siak after the implementation of the SKK filling guidelines. The results of the study show that the percentage of conformity between the SKK format at RSUD Tengku Rafi'an Siak and the Filling Guidelines is 56.25% for completeness and 56.71% for filling format. This is due to the fact that RSUD Tengku Rafi'an Siak has not fully implemented the SKK filling guidelines, and most of the informants interviewed were unaware of the existence of the 2016 SKK Filling Guidelines. The conclusion of this study is that RSUD Tengku Rafi'an Siak has not yet implemented the SKK filling guidelines.
