COVID-19 is a new type of disease caused by a coronavirus, SARS-CoV-2, commonly referred to as the Corona virus. Serum ferritin is an iron-storage protein widely used as an indicator of iron status but can also serve as an inflammatory marker. Macrophages are thought to play a role in the secretion of serum ferritin, and various stimuli such as IL-6 can induce ferritin synthesis. Several studies have reported that hyperferritinemia (ferritin ≥ 500 mg/mL) is associated with worse disease outcomes. The purpose of this study was to describe ferritin levels based on the severity of COVID-19 patients treated at Arifin Achmad Hospital from January to December 2021. The study design was descriptive-quantitative using a cross-sectional approach with a total of 119 samples based on the inclusion and exclusion criteria. The results of this study showed characteristics based on gender of the male group, 65 people (54,6%) and female 54 people (45,4%), with a higher median ferritin value in men (740,60 ng/mL) compared to women (247,65 ng/mL). Ferritin levels based on the severity of COVID-19 consist 21 patients with mild severity, 69 with moderate severity, 23 with severe cases, and 6 people had a critical degree. The highest ferritin levels were observed in patients with severe cases, with a median ferritin level of 1200,00 mg/mL. Ferritin levels tended to be lower in patients with mild cases, with a median level of 128,70 ng/mL. A Kruskal-Wallis statistical test showed a significant association between the severity of COVID-19 and ferritin levels (p-value: 0.001).
