Gambaran Surat Keterangan Kematian di RSUD Petala Bumi Provinsi Riau Pasca Diberlakukannya Pedoman Pengisian Surat Keterangan Kematian Tahun 2016

Death is a natural thing experienced by a human being. Every time a death occurs, a doctor has the authority and responsibility to declare the death by issuing a death certificate. A death certificate is a document containing certain identities with the aim of declaring a person's death. Death certificates have a standard format established by the Ministry of Health. The completeness and filling out of the death certificates format from each hospital play a significant role in health data reporting. However, there are still many instances of incompleteness in the format and filling of the death certificate format from a hospital. The purpose of this study was to determine the description of death certificates at Petala Bumi Regional Hospital, Riau Province after the implementation of the 2016 Death Certificate Filling Guidelines. The research method used is a mixed method with a sequential explanatory design, which involves using quantitative methods followed by qualitative methods to support the quantitative results. This research was conducted by collecting secondary data from death certificates and followed by in-depth interviews with informants closely related to the death certificates. The results of this study obtained 285 samples that met the criteria, with a format completeness percentage based on the guideline book of 56.25% for the old format and 68.75% for the new format. The percentage of format filling was 62.2%. The informant did not know about the existence of a guideline and only used the existing death certificate format from the previous year. The conclusion of this study is that the death certificate at Petala Bumi Regional Hospital has not followed the Death Certificate Filling Guidelines and only uses the death certificate that has existed from previous years.
