Karakteristik rekonstruksi free flap dengan scratch test intraoperasi di rsud arifin achmad riau periode agustus 2023-agustus 2024
The success of free flap reconstruction is measured on the vitality of the transferred flap. A flap assessment method is needed therefore can evaluate flap vitality quickly so that if complications occur, reexploration can be carried out as soon as possible. In order to detect free flap complications early, Arifin Achmad Hospital now uses the intraoperative scratch test technique to assess flap perfusion in the hope of minimizing tissue necrosis. This study was conducted using medical records of patients who underwent free flap reconstruction with an intraoperative scratch test at Arifin Achmad Hospital, Riau Province, from August 2023 to August 2024. The purpose of this study was to determine the characteristics of free flap reconstruction in terms of preoperative, intraoperative, and postoperative aspects at Arifin Achmad Hospital. The research design used in this study was descriptive with a cross-sectional study method involving medical records of 35 patients who underwent 37 free flap reconstruction procedures with an intraoperative scratch test. The data were processed and presented in the form of a frequency distribution table. The results of the study based on the preoperative aspect of 35 patients showed that the largest age group was the elderly (46-65 years) with 13 patients (37.1%), the largest gender was male with 20 patients (57.1%), the largest location of the defect was head and neck with 30 patients (85.7%), the largest etiology was malignancy with 28 patients (80%), smoking status was non-smokers with 20 patients (57.1%), patients without comorbidities with 29 patients (82.9%). The results of the study based on the intraoperative aspect of 37 procedures showed that the most common type of free flap was the ALT free flap with 16 flaps (43.2%), the average length of surgery was 502 minutes, and the average ischemic time was 50 minutes. The results of the study based on the postoperative aspects of 37 procedures showed that perfusion complication occurred in 6 flaps (16.2%), most common type of perfusion complication is venous compromise (50%), majority of perfusion complication appear within 24 hours after surgery (66.6%), salvaging was performed on 4 out of 6 flaps (66.6%), successful salvaging only occurred in 1 of 4 flaps (15%), the average length of patient care was 13 days, and flap vitality was 32 flaps (86.5%).
