This final project is explain about Online Information System Web-Based Report Card at SMA Negeri 1 Dayun Siak. Information System Report Card Online Web Based on SMA Negeri 1 Dayun Siak still use the notes in the ledger in processing the value that Microsoft Excel and written into a book report card that can cause errors in data processing students such as the redundancy of data, causing data generated less effective. It required a Web-based report card system that can avoid it all, equipped with teacher data, student data, the data classrooms, data subjects and stored in a database in order to schedule data subjects and data value well structured. This information system using the programming language HTML, PHP and MySQL as a database. For design the system using a flowchart, context diagrams, charts tiered (Hirarchy Chart), DFD (Data Flow Diagram), data dictionary, and diagrams ER (Entity Relationship).
