Gambaran Indeks Barthel Dan Discharge PlanningPada Pasien Stroke Di Rumah Sakit Umum Arifin Achmad Pekanbaru

Stroke occurs due to focal brain function disorders that can cause death, motor disorders, communication difficulties, bladder dysfunction, swallowing disorders and psychological disorders so that the level of patient independence is disturbed, assessment of independence is required from the time the patient enters until the patient returns home. The purpose of the study was to describe the barthel index and stroke discharge planning at Arifin Achmad Hospital in Pekanbaru. This study uses quantitative methods with descriptive research designs. The research sample was 38 respondents. The instruments used in this study are standard barthel index and discharge planning observation sheets. The results showed that the frequency of barthel index in stroke patients in 38 respondents was mostly in the category of moderate dependency as many as 28 respondents (73.7%), respondents who were at a level of heavy dependence were as many as 9 respondents (23.7%) and respondents who were in the level of mild dependence, which is as much as 1 respondent (2.6%). The discharge planning results in stages 1 to 4 were carried out entirely on all respondents, namely 38 respondents (100%), while the discharge planning in stage 5 was carried out entirely on giving home recipes, control letters, rehabilitation references and health information, namely 38 respondents (100%) but the provision of information is only given orally without using leaflet media, namely 38 respondents (100%). The conclusion in this study is that the barthel index and discharge planning studies have been carried out in stroke patients at Arifin Achmad Hospital. Based on the results of this study, it is expected that barthel index and discharge planning are carried out continuously so as to increase the independence of post-stroke patients.
