Profil Mutasi Gen Reverse Transcriptase Virus Hepatitis B Pada Penderita Hepatitis B Kronik Di Rsud Arifin Achmad Provinsi Riau
Hepatitis B is a liver disease characterized by hepatocellular injury caused by
hepatitis B virus (HBV) infection. Reverse transcriptase is part of the HBV
polymerase enzyme which lacks proofreading activity and is therefore prone to
error during replication. Mutations in reverse transcriptase gene are associated
with the emergence of resistance to nucleos(t)ide analogue antiviral treatment.
The purpose of this study was to determine the HBV reverse transcriptase gene
mutation profile and its relationship to antiviral resistance based on clinical
manifestations and HBV genotypes. This was descriptive study with a crosssectional
approach. The study subjects included chronic hepatitis B (CHB)
patients who attended outpatient clinic at the Department of Internal Medicine
Regional Hospital of Arifin Achmad Riau Province between March to October
2019. The research subjects aged 44,6 ± 14,3 years old (mean ± SD). There were
14 (53,8%) males and 12 (46,2%) females. Based on clinical manifestations, there
were 14 (53,8%) CHB patients without complications, 9 (34,6%) CHB patients
with cirrhosis and 3 (11,5%) patients CHB with hepatoma. The HBV genotype
consisted of 15 (57,7%) of genotype C and 11 (42,3%) of genotype B. There were
no classical mutations in the reverse transcriptase gene associated with antiviral
resistance. However, several non-classical mutations, including putative
mutations S213T and Q215H were found in 1 (8,3%) with CHB patient with
complication, F221Y and N238Q/H mutations in 6 (42,8%) CHB patients without

complication and 5 (41,6%) CHB patients with complication. Further,
pretreatment mutation V224I was found in 8 (57,1%) CHB patients witout
complication and 7 (58,1%) CHB patients with complication. Based on the HBV
genotypes, putative mutations S213T (9%), Q215H (9%), F221Y (100%) and
N238Q/H (9%) were found in genotype B while mutations N238Q/H (66,6%) and
pretreatment mutation V224I (100%) in genotype C.
