Perbandingan Efek Pemberian Propofol Dengan Tiopental Terhadap Relaksasi Otak Dan Respon Hemodinamik Pada Tindakan Kraniotomi Pengangkatan Tumor Supratentorial
Background:
Brain tumor surgeries, specifically supratentorial tumor removal, present
challenges related to brain relaxation and hemodynamic stability during the
procedure. The selection of intravenous anesthetic agents such as propofol and
thiopental is critical due to their varying effects on intracranial pressure, cerebral
blood flow, and hemodynamic parameters. This study aims to compare the effects
of propofol and thiopental on brain relaxation and hemodynamic responses during
supratentorial tumor removal.
Methods:
A randomized experimental study was conducted at Arifin Achmad General
Hospital, Riau Province, involving patients undergoing elective supratentorial
tumor craniotomy. Patients were divided into two groups: the propofol group (1-3
mg/kg body weight, followed by maintenance of 50-100 μg/kg/min) and the
thiopental group (4-6 mg/kg body weight, followed by maintenance of 100-200
μg/kg/min) with continuous infusion until bispectral index 40-60 achieved.
Hemodynamic parameters, including mean arterial pressure and heart rate, were
measured during surgery, and at multiple intervals. Brain relaxation was assessed
subjectively by the neurosurgeon using a four-point relaxation scale.
Results:
Thiopental tends to provide better hemodynamic response, but the differences are
not significant, suggesting clinically similar effectiveness. In mean arterial
pressure, thiopental showed better response in induction (p=0.005) and post
intubation (p=0.023). Thiopental showed lower occurrence of inadequate brain
relaxation score (score 3-4: 10%) compared to the propofol group (score 3-4: 30%,
p=0,331, PR=1,286) although this result was not statistically significant.
Conclusion:
The hemodynamic response in patients undergoing craniotomy for supratentorial
tumor removal who received intravenous thiopental was better than intravenous
propofol and there was no significant difference in terms of brain relaxation
between the two groups.
