Patient transfer, particularly in critical conditions, is a complex process that carries the risk of physiological disturbances, morbidity, mortality, and patient safety incidents. Proper implementation of the transfer process is essential to ensure patient safety and the quality of nursing care. Several critical elements must be considered to guarantee safety during the patient transfer process. This study aims to analyze the relationship between nurses’ knowledge, documentation, and communication with the intra-hospital transfer process at Awal Bros A. Yani Hospital. Methods: This study employed a quantitative research design with a cross-sectional approach. The sample consisted of 160 respondents selected using a total sampling technique. Research instruments included a knowledge questionnaire and observation sheets to assess documentation and communication. Bivariate analysis was conducted using the Chi-Square test, while multivariate analysis used logistic regression. Results: The majority of nurses had a good level of knowledge (75,6%), effective communication (51,9%), and incomplete transfer documentation (58,8%). A total of 66,3% of transfers were reported to have been carried out properly. Bivariate analysis revealed a significant relationship between knowledge and the transfer process (p = 0,001
