Coronavirus disease-19 (COVID-19) is a contagious infectious diasease that has been declared as a pandemic on March 11th 2020 by World Health Organization. Symptoms of COVID-19 can be vary from having no complaints, mild, moderate, severe, critical and even able to cause death. Worsening of COVID-19 diseases is related to an imbalance between the immune system and weak adaptive immunity which can be seen from an increase in the neutrophil-lymphocyte ratio. The neutrophil-lymphocyte ratio is a simple inflammatory marker that is easy to measure, cost effective, reliable and can be used to predict the clinical outcome of various disease including COVID-19. This study aims to determine the differences of neutrophil-lymphocyte ratio of survivors and non-survivors patient with COVID-19 at the Arifin Achmad Regional General Hospital of Riau Province. The design of this study was cross-sectional analytic with a total sample of 292. The result of this study showed that the majority of patients were aged 18-59 years old (76%) and the most gender was female (60,3%). The median value of the neutrophil-lymphocyte ratio of survivors is 3.09 (0.32-42.68) and non-survivors is 7.3 (3.47-44.81). Based on statistical tests, there was a significant difference between of neutrophil-lymphocytes ratio of survivors and non-survivors of COVID-19 patients (p
