Death results in the loss of a person's social and legal status, so an official certificate is required to declare a death. A Death Certificate (SKK) is a letter issued by a hospital/health center that contains the identity, time and place of death of the body. SKK must be made based on the SKK filling guidelines issued by the Ministry of Health in 2016, but there are still many hospitals that have not applied the filling guidelines. The purpose of this study is to find out the description of SKK at dr. Suhatman, M.A.R.S Hospital Dumai City after the enactment of the guidelines for filling out SKK by the Ministry of Health in 2016. The method used is Sequential Explanatory by conducting a quantitative method first and then a qualitative method to deepen the results of quantitative findings. The number of samples in this study was 317 SKK and 9 Informants in in-depth interviews. The results of the study obtained the suitability of the SKK format at dr. Suhatman, M.A.R.S Hospital with the guidelines for filling out the SKK by 50% (8 variables) and the average completeness of filling out the SKK by 86.57%. Doctors, health workers and officers responsible for SKK issuance do not know about the guidelines for filling SKK by the Ministry of Health in 2016. The inconsistency of the SKK format and lack of knowledge about the 2016 SKK guidelines are due to the lack of socialization of the guidelines and hospital evaluation of the SKK issued. Socialization and training are needed in making SKK in accordance with the guidelines.
