Gambaran Biaya Pengobatan Pasien Rawat Inap Community Acquired Pneumonia di RSUD Arifin Achmad Provinsi Riau
Community Acquired Pneumonia (CAP) is a global health problem that can cause high mortality, morbidity, and costs worldwide. The 2014-2018 National Health Insurance (JKN) statistics show that pneumonia is one of the top ten hospitalization cases in Indonesia. This study aims to examine the cost of treatment of CAP patients admitted to Arifin Achmad Hospital in relation to patient severity level. This research method is a descriptive analytic study, with a cross sectional design using secondary data in the form of medical record data and details of the treatment costs of CAP patients hospitalized at Arifin Achmad Hospital Riau Province in 2023. A sample of 326 patients was obtained with the inclusion criteria of patients aged ≥18 years and exclusion criteria, namely CAP patients who died within 24 hours, patients with comorbidities of Human Immunodeficiency Virus (HIV) and tuberculosis. Univariate analysis showed that the highest total medical costs were patients with severity level III, obtained a median of Rp.14,657,287 (Rp.2,360,592 -Rp.221,476,154), with the largest cost component obtained in the cost component of drugs with a median of Rp.5,041,406 (Rp.126,428- Rp.103,654,294), followed by a diagnostic cost component of Rp.3,612,500 (Rp.320,000- Rp.19,307,000). The INA-CBG's claim rates tend to be lower than the real hospital costs. Bivariate analysis using the chi-square test showed that there was a statistically significant relationship between severity level and the patient's PSI score (p=0.024), so the use of the PSI scoring system can also help predict medical costs and INA-CBG's rates that can be claimed by patients.
