Hubungan Penilaian Scratch Test 3 Tahap Intra Operasi Bedah Mikro Terhadap Vitalitas Flap di RSUD Arifin Achmad
Free flap is a tissue reconstruction technique used in trauma and oncological surgery, involving the transfer of tissue along with blood vessels from one site to another. The success of the procedure is determined by the vitality of the flap, or the survival of the transplanted tissue. In 2023-2024, microvascular surgeons at RSUD Arifin Achmad began implementing a three-stage intraoperative scratch test as an additional method for assessing flap vitality. This study aims to compare the success rates of free flap procedures with and without the three-stage intraoperative scratch test. Data were obtained from patient medical records during the periods of January 2021–December 2022 (without scratch test) and August 2023–August 2024 (with scratch test). Analysis showed that the flap vitality percentage was higher in patients undergoing surgery with the scratch test (88.9%) compared to those without the scratch test (74.2%). Salvage procedures in the group without the scratch test were performed on 6 flaps, with 3 flaps maintaining vitality (50%), while in the group with the scratch test, salvage was performed on 4 flaps, with only 1 flap remaining viable (25%). However, statistical analysis using the Chi-Square test yielded a p-value of 0.21, indicating that the relationship between the three-stage intraoperative scratch test and flap vitality was not significant (p>0.05). This study demonstrates that the three-stage intraoperative scratch test is a simple, informative, and cost-effective method that can be used for early detection of flap perfusion disturbances before clinical signs appear.
