Death is the absence of life or the opposite of life. All living creatures will experience death. According to the Joint Regulation of the Minister of Home Affairs and the Minister of Health Number 15 of 2010, all deaths and their causes must be reported. One of the documents required to report a death is the Death Certificate (SKK). In its making, the Ministry of Health issued guidelines for filling out the SKK in 2016. However, many hospitals have not implemented these guidelines. The aim of this study is to describe the implementation of SKK at Arifin Achmad Regional General Hospital, Riau Province, after the enactment of the 2016 Ministry of Health guidelines for filling out the SKK. The research was conducted using a Sequential Explanatory method, which involves a quantitative followed by a qualitative method to deepen the quantitative findings. The total of samples used in this study were 376 SKK and 9 informants. The results showed that the conformity of the SKK format with the SKK filling guidelines was 68.75%, and the average completeness of the SKK filling was 85.08%. Doctors, healthcare workers, and staff were unaware of the 2016 SKK filling guidelines issued by the Ministry of Health. The discrepancies in the SKK format and the lack of knowledge among staff regarding the 2016 SKK guidelines were due to insufficient socialization about the guidelines. There is a need for socialization and training in filling out the SKK in accordance with the guidelines.
