Gambaran Medication Errors Fase Prescribing Di Puskesmas Payung Sekaki Pekanbaru

Medication errors are every preventable occurences that cause improper usage of medicines and cause harm to the patients. Medication errors can happen in every phase from prescribing, transcribing, dispensing, and administration. Prescribing errors is defined as mistakes at giving a medication to a patients and can be a mistake in dosage, strength, route, amount, indication or contra-indication. The purpose of this research is to find out the analysis of Medication errors in Prescribing phase in Payung Sekaki Community Health Care Pekanbaru. This research uses descriptive-quantitative method retrospectively using 180 prescriptions in Payung Sekaki Community Health Care Pekanbaru. Every prescriptions was then evaluated for their completeness based on the information of the prescriptions writes (name, signature, date of prescriptions, dan origin of the prescriptions), patient’s informations (name, age, weight) and drugs’ information (form, strength, dosage, frequency, amount, and time-usage imstruction). Based on this research it was found that 99,8% prescriptiom doesn’t have a doctor’s signatures, 58,3% doesn’t have patient’s weight written, 1,6% doesn’t have patient’s age and unwritten usage frequency happen in 0,19% prescriptions.
