In supply chain activities there is always the potential for risk to arise, so risk management is needed to deal with the causes of risk. PT. Agro Subur Anugerah is one of the organic fertilizer producers whose supply chain activities have the opportunity for risks. Therefore, it is necessary to carry out risk identification, risk analysis, risk evaluation and risk mitigation. Mapping supply chain activities in this research uses the SCOR (supply chain operation reference) model which consists of plan, source, make, deliver and return. The analysis used in this research uses the HOR (House of risk) model which consists of 2 phases. The first phase, namely identifying risk events and risk causes (risk agents), then measuring severity and occurrence levels and calculating the Aggregate Risk Priority (ARP) value to determine the priority of risk causes. The second phase, namely designing mitigation actions and determining mitigation action priorities.
From the results of mapping supply chain activities using the SCOR model, 25 risk events and 22 risk agents were identified. The results of the HOR phase 1 analysis, found 4 priority risk causes (risk agents) for mitigation action, namely disrupted fuel supply (ARP 126.23), engine damage (ARP 121.65), inaccurate application of supplier criteria (ARP 77 .00), and inaccurate quality checking of raw materials (ARP 73.00). There are 6 mitigation actions that can be implemented to handle priority risk agents in organic fertilizer supply chain activities based on calculations in HOR phase 2, namely carrying out regular stock taking (ETDk 378.68), carrying out periodic maintenance (ETDk 364.95), determining supplier criteria (ETDk 304.00), implementing incoming quality control (ETDk 146.80), managing spare parts (ETDk 121.65), and forecasting (ETDk 94.67).
