ABSTRACT
Goods Request and Expenditure Receipt is a proof letter for the request and expenditure of logistics goods from a work unit with a certain type of specification and amount of logistics addressed to the warehouse section with a certain time. PT. Nusantara V Plantation still uses manual methods for recording and approval of Bon through a blank piece of paper by the plant assistant and signed wet by several related parties. Therefore, to collect the signatures completely, it takes 5-30 working days. If all parties are in place, it is possible for the Bon to be immediately signed and approved. But if one, or many parties are not in place, Bon approval is delayed. This causes it is not uncommon for goods to leave the warehouse first because of urgent needs, while Bon has not been approved. So, based on these problems, an application is needed that is expected to make it easier for companies to manage Receipts and Expenditures of Goods from warehouses. This application is implemented into a website, where users can approve Receipts for Requests and Releases of Goods online and gradually. The method used in this research is the prototype method, and uses PHP programming language framework laravel and MySQL as a database in the development of this application. The results of this application's blackbox testing get a score of 100%. As for usability testing using the USE questionnaire got a score of 89.98% for the usefulness aspect, 89.27% for the ease of use aspect, 90.11% for the ease of learning aspect, and 90.93% for the satisfaction aspect. The total value of usability testing of 90.07% reached the Very Decent category on system feasibility.
