Analisis Efek Antiinflamasi Metformin Melalui Perubahan Ekspresi Tumor Necrosis Factor Alfa (TNFΑ) Dan Interleukin 6 (Il-6) Pada Tikus Model Hemoroid
Hemorrhoids are a condition characterized by the dilation of venous blood vessels in the anorectal region, involving inflammation, thrombosis, and bleeding. Anti-inflammatory treatments are crucial in preventing the progression of hemorrhoids. Metformin, primarily used as an antihyperglycemic drug, also has anti-inflammatory properties. Its use in patients with type 2 diabetes shows potential in reducing the risk of hemorrhoids. However, study on the anti-inflammatory effects of metformin on hemorrhoids are limited.
Method: Sprague-Dawley rats divided into six treatment groups. These groups included normal, negative, positif control, and metformin-induced with doses of 3, 9, and 15 mg. Anorectal tissue samples were obtained for qPCR analysis of the expression relative mRNA TNF-α and IL-6 genes. The results were compared among the different groups through statistical ANOVA. Histopathological examination was conducted to assess leukocyte infiltration and venule diameter in the anorectal tissue were conducted and compared across the groups.
Result: This study's findings Aspirin administration reduced IL-6 mRNA expression to 87% of the normal control, while 9 mg metformin reduced IL-6 mRNA expression by 99%. Metformin at 3 mg and 9 mg decreased TNF-α mRNA expression by 99%, which was higher than the 66.7% reduction observed with aspirin. Histopathological examination showed that leukocyte infiltration with 15 mg metformin was 60% of the normal control, whereas aspirin resulted in 173%. The smallest venule diameter was observed in the 15 mg metformin group, being 47% smaller than the normal control, while the positive control showed venules 27.5% larger than the normal control.
Conclusion: Metformin have anti-inflammatory potential in hemorrhoid. Furthermore. study pre-clinical trials and clinical trial phase I is required to evaluate the effectiveness of metformin in treating hemorrhoid..
