Background: Hemorrhoids are a medical condition characterized by inflammation and dilation of blood vessels around the anus and rectum. According to WHO data from 2014, the number of hemorrhoid sufferers worldwide is estimated to reach 230 million people and is predicted to increase to 350 million by 2030. This study aims to evaluate the effects of metformin on the mRNA expression of COX-2 and venule diameter in a hemorrhoid rat model.
Methods: A laboratory experimental study with a post-test only design was conducted on 18 male Sprague Dawley rats induced with hemorrhoids using croton oil. The rats were divided into six groups: normal, negative control, positive control, and three metformin treatment groups with doses of 3 mg, 9 mg, and 15 mg. Measurement of mRNA COX-2 expression was performed using the RT-qPCR method.
Results: Metformin significantly inhibited mRNA COX-2 expression at all dose levels compared to the negative control group. The highest COX-2 expression was observed in the negative control group, while the lowest was in the 15 mg metformin group. Additionally, the venule diameter in the metformin-treated groups was significantly lower compared to the negative control group. However, there was no significant increase in IL-10 expression with metformin administration.
Conclusion: Metformin is effective in reducing mRNA COX-2 expression and venule diameter in the hemorrhoid rat model, indicating its potential as an anti-inflammatory agent in the treatment of hemorrhoids. Further research is required to determine the optimal dose of metformin and assess its long-term effects in humans.
