at the Village office of Palung Raya Village, District Tambang of Kampar Regency. The data collection of the citizens at the Village office still using the Application of Microsoft Office (Word & Excel) and the record book, so the data that has been created is less effective. The information system of citizens’ data collection at the level of this village will help the government instance to collecting the data of the citizens. This system is using a language programs HTML, PHP, and MySQL as the database. The programs of the system are using Framework Yii, Diagram UML (Use Case Diagram, Activity Diagram, and Sequence Diagram). The information system of this citizens’ data collection will be a help in processing the certificate of birth, data of the death, data processing of moving come and the process of making citizens’ report in a way that more efficent.
