Gambaran Kejadian Medication Error Fase Prescribing Di Puskesmas X Pekanbaru
Prescribing error is preventable error, occurs in prescription writing process, may be classified into errors of omission refer to missing essential information in a prescription and errors of commission refer to wrongly written information in a prescription. A prescription should be written legibly, accurately and completely in order to prevent the miscommunication. The aims of this study were to determine prescribing error from occurrence of prescriber, patient and drug information in prescription sheets in Community Health Care. This study was a quantitative descriptive study retrospectively including 178 prescription sheets in Community Health Care X Pekanbaru at Oktober 2016. All prescriptions were evaluated for presence of prescriber information (name, signature, date and room where the prescription from), patient information (name, age, weight) and drug information (drug form, strength, route, doses, frequency, amount of drug to be dispensed, and instruction when the drug taken). The result of this study was signature of prescribers was missing in 98.6% prescription orders, for patient information, children’s weight was missing in 100% prescriptions and based on drug information, the strength of drug was the most drug information that didn’t mentioned in 79.8% of all prescription orders. All of prescription orders were categorized in incomplete prescription order. The doctors in primary health care should write prescription according to PERMENKES RI No 30 Tahun 2014 and make a guideline and intervention about how to make a good prescription.
