With the increasing number of hospitals in Indonesia, the amount of medical waste produced is increasing. Hospital medical waste is categorized as hazardous and toxic waste (B3). The impact caused by waste in health services due to its poor management on the environment can lead to a deterioration in the quality of the environment where health services are located which can disrupt and cause health problems for people living in the environment where health services are located and can cause unsightly aesthetics of the environment so that it disturbs comfort. patients, officers, visitors and the surrounding community. At type B Hospital as a hospital referral center in Riau Province is also inseparable from the problem of B3 waste.
This study aims to analyze the volume of hazardous and toxic waste (B3), the characteristics and management of B3 waste in At type B Hospital, Riau Province. In addition, the purpose of this study is to analyze the social and economic impact of B3 waste on patients and the hospital and to formulate a B3 waste management strategy at At type B Hospital, Riau Province.
This research was conducted at At type B Hospital, Riau Province because At type B Hospital is a referral center in Riau Province. This research is a mixed method research, which is a research step using two approaches in research, namely qualitative and quantitative. Collecting data using observation and interviews. Data analysis was carried out descriptively and using SWOT.
The results of this study are that in 2019, the total amount of waste produced by At type B Hospital in Riau Province was 71,616.2 kg and in 2020, the total amount of waste generated from January - August by At type B Hospital, Riau Province was 73,747.5 kg. . The highest amount of B3 waste in At type B Hospital, Riau Province is medical waste with 53,237.5 kg, followed by sharp object waste as much as 7,194 kg. B3 waste management consists of three stages, namely sorting, transporting and treating waste. Improper waste management can cause inconvenience for patients, health workers, and waste transport officers. There have been no social problems caused by the management of B3 waste and until now there has been no economic value from waste management at At type B Hospital, Riau Province.
Based on the research results, it is known that medical waste sorting is carried out by separating trash cans. Medical waste waste is wrapped in yellow plastic measuring 90x90 cm and put in a trash can labeled infectious waste. Non-medical waste is wrapped in black plastic then put in a trash can labeled non-infectious waste. The waste of human organs is wrapped in yellow plastic. Radioactive waste is put into red plastic. There is conformity with the results of field evaluation observations, namely for the containerization of medical waste using yellow plastic bags and non-medical waste using black plastic bags and sorting sharp waste such as syringes that are inserted into a safety box.
The transportation route at the hospital does not yet have a special route, so it passes through several hospital corridors and canteens, the time of

transportation is also not according to the predetermined schedule, even during the patient's visiting hours. The absence of a special transportation route at the hospital has resulted in the process of transporting medical and non-medical waste through the route of visitors and patients and also inappropriate transportation times, which is still carried out during patient visiting hours, thus damaging the aesthetics of the hospital, spreading disease if garbage falls and is scattered. on the floor. This is not in accordance with the Kepmenkes RI PP No. 101/2014 and Permen LHK number 56/2015, namely that hospitals should have a special transportation route for the process of transporting hospital waste. So that the process of transporting hospital waste does not damage the aesthetics of the hospital and does not cause new diseases for the hospital.
Hospital medical and non-medical solid waste storage is placed in the hospital TPS which is located in the backyard of the hospital which is adjacent to the incinerator. In the process of disposal of hospital waste, especially for medical solid waste, it is carried out by burning it in an incinerator. However, in the field the hospital incinerators often experience over capacity, lack of fuel supply and equipment is damaged. So that often there is a buildup of B3 waste. Meanwhile, domestic waste is transported and processed by the Public Works Agency once every 2 days. The delay in transporting domestic waste is up to 3 days. So that often causes non-medical waste to accumulate.
Based on the results of the SWOT analysis, several B3 waste management strategies were obtained, namely: 1) a separate medical solid waste container system and sufficient land availability to be used as a basis for seizing opportunities to make laws and regulations a reference material in B3 waste management activities, 2) With the existence of an institution Waste managers in hospitals and a separate medical solid waste container system are used as the basis for seizing opportunities for coaching activities from the Pekanbaru City Health Office regarding B3 waste management activities. 3) With the existence of a waste management agency in the hospital, it is hoped that it can write writing near the trash so that visitors can distinguish between the placement of infectious and domestic waste, 4) The existence of financial support from the government is expected to fix the problem of incomplete waste combustion
