This final project aims to design and implement a web-based Shift Operational Daily (SOD) information system at Awal Bros Ahmad Yani Hospital. The system is intended to replace the previous manual reporting process using spreadsheets, which often caused issues such as data entry errors, reporting delays, and difficulties in data management. The research was conducted through stages of observation, interviews, system requirement analysis, design using Unified Modeling Language (UML), implementation with PHP and MySQL, and testing using the Black Box Testing method. The resulting system is capable of recording and managing shift reports in a structured, real-time, and secure manner. With this system, the reporting process becomes more efficient, accurate, and supports faster decision-making by hospital management.
