Gambaran Medication Errors Fase Prescribing Pada Terapi Diare Akut Pada Anak Di Klinik Swasta X Pekanbaru Periode Januari 2015-Desember 2015

Medication errors is any preventable event that my cause or lead to inappropriate medication use or patient harm. The aim of this study is to determine about the representation of medication errors prescribing phase toward acute diarrhea therapy on children at the private clinic X in subdistrict Tampan Pekanbaru. Designe used in this study was descriptive retrospective based on patient medical record. Data collection was conducted during september 2016 until Januari 2017. This study was located at the private clinic X in subdistrict Tampan Pekanbaru with total sampling method. The result showed that the characteristic of acute diarrhea patient (who has medication errors in prescribing phase) based on gender and age are 56,6%, 46,2%. The most of man with the range of age about 5-18 years old. Amount of medication errors in prescribing phase toward acute diarrhea therapy on children is 268 case with involved 377 presrcriptions from 106 medical record. Each of the most prescribing erros types from 377 prescriptions is no number of medicine (48,54%) and from 106 medical record is no weight patient (65,09%).
