Modification of The Creating Opportunities for Parent Empowerment (COPE) programme is a parent empowerment programme to improve parental knowledge, involvement, and skills in pediatric care, especially in the PICU. The aim of this study is to identify the effect of a modified COPE program on reducing parental stress in the PICU. Methods: This is a quantitative study using a quasi-experimental design with a control group and an experimental group. The study involved 56 participants selected through simple random sampling based on inclusion and exclusion criteria. Data were collected using the Parental Stress Scale (PSS). Results: The study showed that most parents in the PICU were in early adulthood (20–44 years), mainly mothers, from the Malay ethnic group, and had a senior high school education level. In the experimental group, most parents experienced severe stress before the intervention (89.3%), which decreased to mild stress (100%) afterward. Meanwhile, in the control group, severe stress remained dominant (92.9%) both before and after the intervention. The statistical test used was the Mann-Whitney test, which showed a p-value of 0.000 < 5% (p < 0.05). This indicates a significant difference in stress reduction between the experimental and control groups after the modified COPE program intervention. Conclusion: Modification of the COPE programme can reduce parental stress in the Pediatric Intensive Care Unit (PICU), and is recommended to be carried out by nursing staff as an important program to reduce stress in parents of patients in the PICU.
