The importance of reporting Patient Safety Incidents (PSI) as an effort by healthcare workers to improve patient safety. Patient safety mentorship is one of the efforts to enhance nurses' knowledge and motivation in reporting incidents so that similar or potential incidents can be prevented. Objective: this study aims to determine the effectiveness of patient safety mentorship on the knowledge and motivation of nurses in reporting patient safety incidents. Method: this type of research is quantitative research with a quasi-experimental design (one group pre and post test intervention) without a control group, involving a sample of 96 respondents selected using purposive sampling techniques. The instrument used to assess knowledge and motivation is a questionnaire developed by the researchers after undergoing validity and reliability tests. The validity test results showed a calculated r of 0,435-0,832 > table r (0,361). The reliability test results for the knowledge variable had a Cronbach’s Alpha value of 0,856, while the motivation variable had a value of 0,838. The data was analyzed using bivariate analysis with a paired t-test. Results: The demographic characteristics of 96 respondents show that the majority of respondents are in the age range of 36-45 years (42%), have a nursing education (81%), have worked for more than 10 years (39%), are civil servants (52%), and have received training (91%). This study indicates that there is a significant influence of patient safety mentorship on nurses' knowledge and motivation in reporting incident occurrences, with a p-value (0,000) < alpha (0,05). Conclusion: patient safety mentorship is effective in increasing the number of Patient Safety Incident Reports (IKP) from three reports to thirty six patient safety incident reports (IKP) that occurred at RSUD Selasih.
