Information Systems in an agency plays a very important role for every company and government agencies and education, because it can simplify the operational activities in various fields. The field of the company, the use of information systems both in the system of certification of employees is one of the process in providing information and obtain information to each employee related companies. Currently the delivery of information is done manually, the purpose of this system is to simplify the process of registration and manage the value of certification and control the data of certification documents to be used by leaders and employees. Making this information system using programming language VB.NET and MySQL as database. While in the design using Data Flow Diagram and Physical Entity Relationship Diagram.
