Introduction:
Pulmonary mycoses as secondary infections contribute to mortality and morbidity of 1.6 million deaths annually. The use of fungal culture specimens can affect the difference in positivity of fungal culture. Accurate specimen selection will help accelerate the diagnosis of pulmonary mycosis so as to improve the quality of life in patients with thoracic cavity malignancy.
Method:
This study is an analytical observational using a cross-sectional approach conducted from August 2024 to January 2025. Samples were collected by consecutive sampling and analyzed through SPPS and presented in distribution tables and diagrams.
Result:
Fungal culture positivity was found in 77.19% with the highest positivity by sputum (57.89%), bronchial washing (56.14%) and tissue (42.11%). There were significant differences in positivity rates between test specimens with the highest positivity of Candida sp from sputum (49.12%) and Aspergillus sp from tissue (28.07%). The discordance of fungal culture results (57.89%) supported the difference in positivity rates of fungal culture from three test specimens. Tissue specimens had the highest accuracy for identifying all fungal species especially Aspergillus sp and bronchial washing was recommended for detecting Candida sp. Chest pain and bronchoscopic visualization had a significant related to the differences in fungal culture positivity.
Conclusion:
Sputum, bronchial washing and tissue had different levels of fungal culture positivity with discordance of fungal culture findings between test specimens. Tissue specimens had the highest accuracy for Aspergillus sp positivity, and bronchial washing recommended for Candida sp positivity. Chest pain and bronchoscopy visualization contributed to the differences in fungal culture positivity.
