World Health Organization (WHO) elaborates stroke as disorder of brain functional which features a sudden onset with clinical manifestasions such as disorder of the cerebral functions, both focal and global. 500.000 of Indonesian had stroke, and about 25% died, the rest are had mild to severe disablement. Mean platelet volume (MPV) is an indicator of platelet reactivity. Platelets play an important role in the pathophysiology of ischemic stroke. This research is aimed to know the determine representation of MPV in severity of acute ischemic stroke based on criteria NIHSS in RSUD Arifin Acmad Riau Province in the period of Desember 2016–February 2017. The result showed normal MPV are found in acute ischemic stroke patients with NIHSS criteria were as many as 22 people (48.88%), a high MPV values are found in patients with ischemic stroke with NIHSS criteria weigh as much as 3 people (6.66%). In conclusion there are representation of MPV in severity of acute ischemic stroke based on criteria NIHSS.
