This final project discusses a web-based fertilizer supply information system at the Muara Mahat Sejahtera KUD (Village Unit Cooperative) located in Muara Mahat Baru Village, Kec. Tapung, Kab. Kampar. Cooperatives in Muara Mahat Baru Village are one of the companies that serve the community in managing and fulfilling community demands, one of which is fertilizer for oil palm plantations. Suppliers are an important factor in providing products for relatively large needs for resale. A web-based fertilizer inventory information system can assist in managing goods. Management of fertilizer suppliers is carried out electronically. This web-based fertilizer inventory information system was created using PHP and MySQL. The purpose of making a web-based fertilizer supplier information system is to help Muara Mahat Sejahtera KUD in managing fertilizer purchases to suppliers.
