Pengalaman Tenaga Kesehatan dan Pasien Dalam Pelaksanaan Discharge Planning Pada Pasien Gagal Jantung
Introduction: Ineffective discharge planning results in in continuity of patient care at home. This condition results in worsening patient conditions, so that patients return to health services with the same medical history or new, more dangerous complications. This study aims to explore the experience of healthcare workers and patients in the implementation of discharge planning on heart failure patients. Method: This study uses a phenomenological study design with a qualitative interpretative approach. The participants involved were ten health workers who work at Prima Hospital Pekanbaru and seven heart failure patients who were planned to go home. Participant recruitment was carried out using the purposive sampling method. Data collection used was in-depth interviews and data analysis used the Colaizzi analysis stages. Results: There are four themes that are interrelated with the phenomena studied, namely: information on discharge planning received by the patient, specific education for heart failure patients, the challenges faced by healthcare workers in filling out the discharge planning form, and important components of discharge planning for heart failure patients. Conclusion: The experiences of healthcare workers and patients in the implementation of discharge planning in this study indicate that there are still shortcomings that are a concern for healthcare workers and expectations from patients. The themes identified in this study can contribute to the development of discharge planning forms for heart failure patients. Therefore, implications of this study for healthcare services can reduce the recurrence rate of heart failure patients.
