Hubungan usia dengan invasi miometrium pada kanker endometrium berdasarkan ekspresi reseptor estrogen
One of the malignancies that frequently affects the female genitalia is endometrial cancer (EC). Endometrial cancer is classified into type I (estrogen dependent) and type II (non-estrogen dependent) according to Bokhman's classification. The role of estrogen in EC pathophysiology can be observed through estrogen receptor expression assessed by immunohistochemistry. Positive estrogen receptor expression indicates the possibility of hormonal therapy and is associated with a favorable prognosis. Myometrial invasion is an important factor in determining prognosis and management. This study aims to determine the relationship between age and myometrial invasion in EC based on estrogen receptor staining at RSUD Arifin Achmad, Riau Province, from January 2019 to October 2024. This study is an analytical observational study with a cross-sectional approach complemented by laboratory analysis. The study population includes all medical records of post hysterectomy EC patients from January 2019 to October 2024, with a sample size of 44 patients. Paraffin blocks were analyzed using immunohistochemistry with estrogen receptor antibodies and assessed using the Allred Score. Most EC patients showed positive estrogen receptor expression (79.5%), were 40-49 years old (31.8%), and had ≥50% myometrial invasion (61.4%). Bivariate analysis showed a significant relationship between age and myometrial invasion when estrogen receptor expression was not considered (p=0.042). However, there was no significant relationship between age and myometrial invasion in either estrogen receptor positive (p=0.109) or estrogen receptor negative (p=0.405) patients. Age influences myometrial invasion when estrogen receptor expression is not considered, but when categorized by estrogen receptor expression, age does not affect myometrial invasion.
