Insidensi Dan Gambaran Klinis Pasien Nyeri Sentral Pascastroke Di Rsud Arifin Achmad Provinsi Riau Periode 2023–2025
Central post stroke pain (CPSP) is a common yet often underdiagnosed complication of stroke that markedly affects survivors’ quality of life. This study aimed to determine the incidence and describe the clinical and pain characteristics of patients with CPSP at Arifin Achmad General Hospital, Riau Province, during 2023–2025. A descriptive cross-sectional design was applied involving 270 stroke patients who met the inclusion criteria. CPSP was identified based on the diagnostic criteria of the Indonesian Neurological Association (PERDOSSI, 2023), and pain characteristics were assessed using the Indonesian version of the Douleur Neuropathique 4 questionnaire (DN4-Ina). The results showed that 44 of 270 stroke patients (16.3%) experienced CPSP. Most were under 60 years old (72.7%), male (68.2%), and had ischemic stroke (90.9%). Lesions were predominantly located in extrathalamic regions (63.6%) with right-sided laterality (40.9%). The median pain onset was 3 months, with a median DN4 score of 4 (range 4–6). Most patients reported persistent pain (68.2%) of moderate intensity (54.5%), and burning sensation was the most frequent symptom (75%). These findings indicate that CPSP has a considerable prevalence with distinctive neuropathic features, emphasizing the importance of early recognition to improve pain management and quality of life among stroke patients.
