Komunikasi terapeutik perawat dalam memberikan motivasi kepatuhan pasien hemodialisa remaja di RSUD Arifin Achmad
According to the World Health Organization (WHO), in 2019, patients with chronic kidney disease accounted for 15% of the global population and caused 1.2 million deaths. In 2020, there were 254,028 recorded deaths, and in 2021, the number of cases exceeded 843.6 million. In Riau Province, data from Arifin Achmad Regional General Hospital show a yearly increase in mortality due to chronic kidney disease. One of the efforts to reduce this number is by improving patients’ adherence to hemodialysis therapy. Adolescents, as a vulnerable group, require a supportive communication approach to stay motivated in undergoing routine therapy.
This study aims to analyze the role of nurses’ therapeutic communication in motivating adherence to therapy among adolescent hemodialysis patients at Arifin Achmad Regional General Hospital. The research used a descriptive qualitative method with eight informants consisting of the head nurse, three nurses, and four adolescent patients. Data were collected through in-depth interviews, observation, and documentation, and analyzed using the Miles and Huberman model, with data validity tested through source triangulation.
The results showed that therapeutic communication was carried out in four stages: pre-interaction, orientation, working, and termination. The pre-interaction stage involved nurses’ physical and emotional preparation. The orientation stage focused on building trust and comfort. The working stage served as the core of communication through emotional support, motivation, and clear explanations of therapy procedures. The termination stage involved evaluation and reinforcement of patient motivation for the next session. Therapeutic communication incorporated both verbal (simple language, gentle tone) and nonverbal (smile, eye contact, empathetic expression) elements.
This study concludes that nurses’ therapeutic communication plays an essential role in enhancing the motivation and adherence of adolescent hemodialysis patients at Arifin Achmad Regional General Hospital.
