Kesesuaian Penggunaan Antibiotik Empiris Dengan Panduan Penggunaan Antibiotik (Ppab) Pada Pasien Sepsis Di Intensive Care Unit (Icu) Rsud Arifin Achmad Provinsi Riau Tahun 2023-2024
Sepsis is a life-threatening medical emergency that requires timely and appropriate empiric broad-spectrum antibiotic therapy. Antibiotic Stewardship Guidelines (ASG) and Antimicrobial Stewardship Programs (ASP) play a critical role in ensuring rational antibiotic use. This study aimed to evaluate adherence to ASG in empiric antibiotic therapy among sepsis admitted to the ICU of RSUD Arifin Achmad Provinsi Riau from 2023 to 2024. A retrospective descriptive study was conducted by collecting data on the type, dose, and frequency of empiric antibiotics, which were then compared against ASG and correlated with clinical outcomes, including recovery and mortality. The results showed that Meropenem was the most frequently used antibiotic (55,3%), followed by Ceftriaxone (12,8%) and Levofloxacin (10,6%), with monotherapy being predominant (71,4%). Adherence to ASG regarding antibiotic choice was observed in only 21,4% of cases, while 78,6% were non-adherent. Among adherent cases, 83,3% received optimal dosing and 61,1% optimal frequency. Recovery rates were higher in patients receiving ASG-adherent therapy (37,5%) compared to non-adherent cases (19.4%), although mortality remained high across both groups. Adherence to antibiotic stewardship guidelines in empiric therapy for sepsis patients in this ICU was suboptimal, particularly in antibiotic selection, yet adherence was associated with improved clinical outcomes. These findings highlight the need to strengthen ASP implementation and enforce more rigorous evaluation of empiric antibiotic therapy to optimize rational antibiotic use and improve patient outcomes in sepsis.
