Background: Breast cancer is the second leading cause of cancer-related death among women worldwide. Approximately 75% of cases are diagnosed in women over the age of 50, making the postmenopausal group the population with the greatest disease burden. Evidence indicates that hypercholesterolemia independently increases aggressiveness and metastasis in postmenopausal breast cancer. This study aimed to analyze the association between lipid profile and the occurrence of metastasis in postmenopausal breast cancer patients at Arifin Achmad General Hospital, Riau Province.
Methods: This retrospective cohort study included postmenopausal breast cancer patients diagnosed in 2018–2019 and followed for 5 years. Subjects were divided into two groups: 62 patients with metastasis and 62 patients without metastasis.
Results: Total cholesterol was significantly associated with metastasis (p = 0.009; OR = 8.017). In contrast, triglycerides (p = 0.295; OR = 1.190), HDL (p = 0.305; OR = 0.679), and LDL (p = 1.000; OR = 0.228) showed no significant association. Patients with elevated total cholesterol levels had more than an eightfold increased risk of metastasis compared to those with normal levels. Linear regression analysis revealed that the lipid profile as a whole contributed 5.1% to the risk of metastasis in postmenopausal breast cancer patients.
Conclusion: Total cholesterol may serve as a potential biological indicator for assessing the risk of metastasis in postmenopausal breast cancer patients.
