Introduction:Free flap reconstruction is a cornerstone procedure in managing complex defects caused by trauma or malignancy. Flap success is highly dependent on tissue viability, making postoperative monitoring essential. The Three-Stage Scratch Test (STTT) was developed as a simple method to detect early flap failure, but its clinical effectiveness remains uncertain.
Objective:To evaluate the association between STTT implementation and free flap survival rates at Arifin Achmad General Hospital, Riau Province.
Methods:This was an observational analytic study with a cross-sectional design using medical record data of patients who underwent free flap reconstruction between October 2016 and July 2019. A total of 191 cases were included and analyzed according to age, sex, comorbidities, flap type, length of stay, salvage procedures, and STTT application. Bivariate analysis was performed using Chi-Square and Fisher’s Exact Test.
Results:Of the 191 patients, 111 (58.1%) were monitored with STTT and 80 (41.9%) without STTT. The overall free flap success rate was 87.4%. Success rates were 86.5% in the STTT group and 88.8% in the non-STTT group. Statistical analysis showed no significant association between STTT application and tissue survival (p = 0.574).
Conclusion:STTT did not improve free flap success compared to conventional monitoring. Clinical assessment remains the primary standard, while STTT may be considered only as an adjunct in resource-limited settings.
